- A way to contact you for clarification and/or notification that records are ready.
- Be as specific and detailed as possible in your description of the records you seek. Failure to provide sufficient information to identify the record(s) you seek may cause delays in satisfying your request.
- Attach additional information, if necessary, to assist us in locating responsive records.
- You can request records to be made available for review in-person, photocopied/printed, or emailed. There is no fee to review records; the fees for electronic or paper copies are in our fee schedule. Please specify in your request how you would like to review/receive your records.
